Self Assessment

We use up to four fields for identification.

In order to provide you complete anonymity and still be able to compare yourself to your peers*, we provide you with four fields to complete.

1. A free-form text field for you to enter a personal ID of your choice. It will appear on charts. We recommend you use a five digit identifier such as: d4EaH
   You choose this id.

2. A team code that is unique and represents the smallest group of people that you work with.
   Provided to you by your assessment coordinator.

3. (Optional) A code that represents a broader group of people that you and your 'team' work with, such as a department, a chapter or a guild.
   Provided to you by your assessment coordinator.

4. (Optional) A code that represents an even larger or different cross section of the company organisation.
   Provided to you by your assessment coordinator.
